The USGS Patuxent Wildlife Research Center's Bird Banding Laboratory provides forms for reporting banded birds online, or you can simply call in your sighting at 1-800-327-BAND. The site also has a wealth of other information about bird banding. NYSOA has a very useful page with information on reporting banded birds especially for several bird-banding projects of interest to New York birders.
